Helping Patients with Administering Warfarin - Colorado Springs

The purpose of Warfarin Center is to assist our providers in managing patients who are taking the anti-coagulant drug warfarin (Coumadin). Warfarin is a difficult drug to dose due to considerable variability with interpatient and intrapatient response, lack of dose/response relationship, and significant dose/response lag time. Pharmacists in the Warfarin Center have received special training and work under a protocol developed to assist providers in obtaining optimal anti-coagulation therapy and to avoid adverse drug reactions. Patients must be referred to the Warfarin Center by their Peak Vista provider.
