We can help you sign up for health care services. Whether you are new to Peak Vista or need help with Medicaid or other programs, our team is here to guide you.

How to Enroll

We know the process can feel confusing. That’s why we walk with you from the first call to your final approval.

Here's how it works:

  • Step 1: Call us or request an appointment online.
  • Step 2: Fill out a patient packet (you can download below).
  • Step 3: Meet wih an enrollment specialist.
  • Step 4: Submit your paperwork.
  • Step 5: Get approval (usually within 1 - 48 days)

What to Bring With You

Bringing the right information helps us help you faster. Here’s what you’ll need for your appointment:

  • Birthdates and names of everyone in your family
  • Photo ID (driver's license or school ID)
  • Proof of income (a paycheck stub or tax form)
  • Your address and phone number
  • Any insurance cards you already have
  • Social Security Number or immigration documents (if you have them)

To save time, fill out new patient forms before your visit.

This helps your appointment go faster and smoother. You can download the forms for adults and children below. Forms are available in English and Spanish. Bring the completed packet with you to your appointment.

Here are answers to some common questions. If you’re not sure about something, ask our staff when you arrive, or contact our Customer Service Center at (719) 632-5700.

  1. Do you offer virtual (online) appointments for enrollment?
    Yes! You can talk to our Enrollment team over the phone or online. We’re here to help you without needing to visit in person. Call us at (719) 344 – 6889.
  2. What paperwork do I need to bring?
    To sign up, bring a photo ID, proof of income, Social Security or immigration papers, and any insurance cards you have. Review the full list of documents in the “What to Bring with You” section above for details.
  3. How long does enrollment take?
    Most visits take about 45 minutes. It might take longer if you’re applying for Medicaid or if you are missing the right papers.
  4. What if I don't have insurance?
    That’s okay! You can still come to Peak Vista. We offer:
    • Help applying for Medicaid and CHP+
    • Discounts based on your income and family size (called a "sliding scale")
    • Help finding the right plan for you
  5. Can I get care before my insurance is approved?
    Yes. If your insurance is still being processed (like CHP+), you can still get care at Peak Vista. You may have to pay a small fee later. Call us to ask what your visit might cost.
  6. What is Medicaid, CHP+, and CICP?
    These are programs that help pay for health care if you don’t have insurance. We’ll help you apply for the one that fits best.
    • Medicaid: For low-income individuals and families
    • CHP+: For children and pregnant women
    • CICP: For Colorado residents without insurance
  7. What is health care reform?
    Health care reform means changes to health care laws that help more people get care. Our Enrollment staff can explain:
    • What insurance plans are available
    • How you may quality for help paying for care
    • What kind of coverage your family needs
  8. What if I forget my appointment?
    We’ll send you a reminder by phone or text. If you miss it, just call us to reschedule. We know life gets busy!
  9. Can I walk in, or do I need an appointment?
    You can do either! Appointments are best, but walk-ins are welcome too. Some locations may be busier than others, so there may be a wait for walk-ins.
  10. Can I get help even if I'm not a Peak Vista patient?
    Yes. Our Enrollment Services are open to everyone. You don’t need to be a patient here to get help with Medicaid, CHP+, or CICP. We’re here to help everyone in the community.
