Patient Portal

• Request prescription/medication refills, medical records, and appointments
• View lab/test results, personal health information, and medical information
• Update your account information
• Review and pay bills

• Access the patient portal by clicking the button below or by going to peakvista.org/portal.
• Click “Let’s Get Started.” Enter your email address and create a password when prompted.
• When asked for your pin, select “No.”
• Complete the process by confirming your email address for future logins.

Once you have created your account, you can access the Patient Portal online or through the mobile app. Tap the App Store or Google Play buttons below to download the app to your phone.
