Our Warfarin Center is designed to help our providers manage patients who are taking the anticoagulant drug Warfarin (Coumadin). Warfarin is a difficult drug to dose, due to considerable variability with interpatient and intrapatient response, lack of dose/response relationship, and significant dose/response lag time.
The pharmacists in the Warfarin Center have received special training to work under a specific protocol to help doctors obtain the best anticoagulation therapy, and to avoid adverse drug reactions. Patients must be referred to the Warfarin Center by their Peak Vista provider. For more information, please call Warfarin Center at .
