Benefits Summary

Our employee benefits are an important part of the total compensation package received by our employees. Peak Vista pays a portion of the medical and dental premiums for employees and their spouse/dependents. The following is a summary of the benefits offered at Peak Vista:

Medical Insurance Plans

PVCHC offers three medical plan options through Anthem BlueCross BlueShield: the PPO base plan, the PPO buy-up plan, and the HSA-compatible high-deductible health plan (HDHP).

Dental Insurance Plan

PVCHC offers a dental insurance plan through Delta Dental of Colorado.

Voluntary Vision Insurance Plan

PVCHC offers a voluntary vision insurance plan through EyeMed. This plan is 100% employee paid.

Retirement Savings Plan

PVCHC sponsors a 403(b) retirement savings plan that allows employees to save for retirement on a pre-tax (traditional) and/or after-tax (Roth) basis. The plan also provides an employer match after the employee completes one year of service. The match is equal to 100% of the employee’s contribution, up to four 4%, and is immediately 100% vested to the employee. All employees are eligible to participate on their date of hire.

Time Off Benefits

Paid Time Off (PTO):
PVCHC provides PTO for eligible employees that can be used for holidays, vacation, personal leave, illness, etc. An employee's accrual is based on hours worked.

Holidays:
These are the holidays PVCHC observes: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day (and the day after), Christmas Eve (closed at noon), Christmas Day, and New Year's Eve (closed at noon).

Other Leave Benefits

PVCHC also offers other leave benefits including Personal Leave of Absence, Jury Duty, Bereavement, FMLA, Military Leave, etc.

Other Benefits

Life and Accidental Death & Dismemberment (AD&D) Insurance:
Term life and AD&D insurance of 1.5 times an employee’s annual compensation, up to $200,000. PVCHC pays 100% of the cost of coverage.

Long-Term Disability (LTD) Insurance:
LTD covers 60% of an employee’s monthly compensation, up to $7,500, if an employee has a qualifying disability lasting longer than 90 days. PVCHC pays 100% of the cost of coverage.

Ability Assistance Program:
The Ability Assistance Program offers personal, confidential guidance and counseling for employees and their dependents at no cost.

Health Savings Account (HSA):
If you enroll in the PVCHC high-deductible health (HDHP) plan, you may be eligible to open and fund a health savings account(HSA). As HSA is a personal healthcare savings account that you can use to pay out-of-pocket healthcare expenses with pre-tax dollars. Your contributions are tax free, and the month remains in the account for you to spend on eligible expenses no matter where you work or how long it stays in the account.

Medical and Dependent Care Flexible Spending Accounts:
FSA plans allow employees to save for qualified health and/or dependent care expenses for themselves and/or their spouse and dependents.

Supplemental Insurance:
Eligible employees may apply for voluntary supplemental insurance (group term life, accident, critical illness, and short-term disability) for themselves and/or their spouse and dependents.

Additional Employee Benefits:
PVCHC offers other benefits including a wellness program, prepaid legal services, health education classes, tuition reimbursement, and employee discount programs.

When your application has been successfully submitted, you will receive an email confirming that we have received your application.

Our job board support team can help resolve any technical questions or system errors that may occur. They can be contacted, at support@applicantstack.com.

Because each hiring manager has a different time frame for filling a position, there is not a definite time when we can guarantee that you will hear back. If a manager is interested in interviewing you, you will be contacted by phone. If you are not being considered, or the position has been filled, you will receive an email. You do not need to apply for a position more than once.

Yes, you can apply with a paper application at our Administration building. However, applying online is the fastest, most efficient way to get your application to one of our hiring managers. If you have already applied for a position online, you should not submit another paper application for that same position.

We may consider candidates for positions that they have not specifically applied for. However, we recommend that you apply for each position that interests you, even if they have the same job title. There are often multiple openings under the same title, but each has a different position number.

If a position is no longer posted on our website, the hiring manager is no longer accepting applications.

We typically receive dozens of well-qualified candidates for each position. Unfortunately, we cannot hire, or even interview, every applicant who applies. Here are some things to consider when applying for a position:

  • Do your qualifications match the desired qualifications for the position? Note that some positions may require, or prefer, a candidate to be bilingual due to the business requirements of our organization.
  • Is your application customized to fit the position? Make sure that your relevant qualifications are easy to find; please don’t assume the manager knows what job duties you performed in other positions. 
  • We recommend including a cover letter specific to the position for which you are applying.

Provider FAQs

We employ all providers under contracts with 1 year terms. Providers also must complete credentialing once a contract is signed; this must be finalized before employment begins. On average, the credentialing process takes approximately 90 days.

Yes, because we are a Federally Qualified Health Center (FQHC), all our health center sites are approved by the NHSC and CHSC for loan repayment program for awarded recipients.

Depending on the candidates' schedule, an on-site visit can last 4-8 hours and will usually include an interview with the hiring manager and VP of Medical and Dental, tour of selected Peak Vista facilities, a benefits briefing and job shadowing with providers. Lunch or dinner is usually included so that you may meet and talk with our current providers in a casual setting.

No. While applicants need not be Board Certified, we require applicants to be board eligible, and Board Certification must be complete, before the start of employment. A valid Colorado license is also required before the start of employment.
