Your health is connected, and so is your care. Peak Vista offers medical, dental, and behavioral health services (often in one location). This makes care more complete, convenient, and personal.

Providers communicate and collaborate to give you consistent, coordinated care—sometimes even connecting you with more than one type of provider in the same visit.

For example, if you tell your medical provider your tooth is hurting, they can often connect you with a dentist the same day—no extra trip, no extra paperwork. The same is true if you need behavioral health support. This teamwork helps you get the right care faster and makes it easier to stay on top of your health.

Our Medical Services

At Peak Vista, we provide care for your whole family —  from everyday needs to long-term health support. Our services include:

What to Know Before Your Visit

Primary care is your main medical care. It includes check-ups, treatment when you are sick, and help with long-term health needs. Primary care helps you stay healthy by treating most health needs, finding problems early, and guiding you to the right care—so you don’t have to rely on the emergency room for every concern.

At Peak Vista, we are here to build a lasting relationship with you and support your health every step of the way.

  • We offer care for children, teens, and adults.
  • We help with everyday health needs and long-term health problems.
  • We also support your overall well-being, including mental health and social needs.

Primary care providers can be: Medical Doctors (MD), Doctors of Osteopathic Medicine (DO), Nurse Practitioners (NP), and Physician Assistants (PA).

Peak Vista’s Care Coordination team makes health care easier by helping you:

  • Schedule and manage appointments: See your doctor or specialist when you need to.

  • Access to resources: Transportation, chronic care programs, and other support.

Whether it’s your first time or you’ve been with us for years, we’re here to support you. Here’s what you can expect during your visit:

  • Friendly staff at the front desk will help you check in and answer any questions you may have.
  • Time to talk with your provider about your health, your concerns, and what matters most to you. We want to hear your story and help you find answers.
  • You may also see a dental and behavioral health provider during your visit if your care team can help with more than one need at the same time.
  • A clear plan for your care, including next steps, follow-up appointments, and any tests or treatments you may need.
  • Support for your whole health, including help with insurance, rides to your visit, language services, and more.

How to Get Care

Find a health center close to home. Peak Vista has many locations across the Pikes Peak and Eastern Plains regions. Explore our list of health centers below to find medical care nearest to you.

You can see a provider from your home using your phone or computer. Telehealth is safe and easy. You can talk to your provider without coming into the health center. It’s great for follow-ups or questions, saves time, and helps you avoid travel.

Do you have questions about our care? Read our answers below.

  1. Do you also offer dental and behavioral health care?
    Yes. Peak Vista offers medical, dental, and behavioral health services. Many of our locations have all three services in the same building, and our providers work as a team. This means if you need dental or behavioral health help, we can often connect you right away—sometimes during your medical visit.

  2. How do I make or change an appointment?
    Call us at (719) 632-5700 or use our online request form. We're happy to help you schedule, cancel, or move your visit.

  3. Who can help me manage my care?
    Our Care Coordination team can! They help with scheduling, referrals, education, and connecting you to important resources like transportation and chronic care support.

  4. What if I don't have health insurance?
    We offer care for everyone. You may qualify for discounts based on your income and family size. Call us to learn more.

  5. Can I bring my child or family member to the appointment?
    Yes! Your family can come with you. You can bring a child, parent, or caregiver if you want support. We also have Well-Child Waiting Areas at three Peak Vista campuses: Academy Campus, International Circle Campus, and Union Campus.

  6. What should I bring to my visit?
    Please bring a photo ID, your insurance card (if you have one), a list of the medicine you take, and any questions you want to ask your provider.

  7. Do you offer services in Spanish or other languages?
    Yes. We have staff who speak Spanish and can provide interpreters for many other languages. Just tell us what you need. We also have many forms and handouts in Spanish.

  8. Can I see a provider without coming in person?
    Yes. You can see a provider from home using your phone or computer. This service is called Telehealth, which we offer for medical care, behavioral health care, and dental care (Teledental). Ask your health center if it's a good choice for you.

  9. What if I need to cancel or reschedule my visit?
    If you need to cancel or change your visit, please call us as soon as you can. We will help you pick a new time that works for you.

  10. How much will my medical visit cost?
    The cost of your visit depends on your insurance and income. We take Medicaid, Medicare, and more insurance plans. We also offer a sliding fee scale program if you qualify.

We're here to help, even if you do not have insurance. If you’re worried about paying for a visit or not having insurance, let us know. We have options available to make sure you get the care you need.

  • We offer a sliding fee scale based on income and family size
