DDHC serves individuals with intellectual and developmental disabilities (IDD), such as autism, brain injury, or cognitive delay. Patients may qualify if they receive services through The Resource Exchange (TRE) or meet specific disability criteria.
Transition planning typically begins around age 17 to help families move smoothly into adult care. Our team works closely with EFMP and Enrollment Services to guide families through that process.
