Patient Guidelines

Our team is available to help you with any concerns you may have regarding our programs or services. If your concern is not resolved to your satisfaction, you have the right to file a complaint with the Colorado State Board of Medical Examiners or the Colorado State Board of Dental Examiners. Upon request, we will provide you with these organization's contact information.

To ensure fair and equal treatment of all our members, Peak Vista has Patient Rights and Responsibilities posted in all centers. Please take a moment to become familiar with them. Members may be discharged from our centers if guidelines are not followed. This includes, but is not limited to, inappropriate behavior and missing three appointments. Please be aware that any missed or late appointments with a specialist counts as two missed appointments. Language translation, interpretation services and deaf signers are available upon request.

Make the Most of your Visits

We strongly believe in open, two-way communication between our patients and providers. By partnering with your provider, you can help choose care options that best fit your values, beliefs and life. 

We encourage you to ask your provider, or other member of your care team, any and all questions you may have − as many times as you need. We never want you to feel embarrassed if you do not understand something. Getting your questions answered, and the clarification you need, can be as simple as following our helpful tips prior to your visit, as well as asking these three questions:
