How do I become a Peak Vista patient?

Becoming a new patient at Peak Vista is easy.

You can conveniently begin the process of becoming a new patient by submitting your New Patient Registration Form online. Following the submission of your form, we will contact you as soon as possible.

If you prefer to complete this form in person, please visit us at one of our many Enrollment Services locations; our knowledgeable Specialists are available to listen and answer your questions. We look forward to welcoming you as a new patient! Learn more about the enrollment process.

 

What do I need to enroll?

Some basic paperwork is required to enroll. Learn what to bring with you to your appointment.

 

How much does it cost?

Unemployed and uninsured individuals are welcome, as well as those with private insurance, Medicaid or Medicare. If you do not have insurance, the fee for a visit is based on the size of your family and your total income and will be assessed using our sliding scale.

 

When will I be able to make my first appointment?

When you enroll, you will be asked to make your first medical, dental or behavioral appointment so that you may get to know your physician / provider. For all appointment inquiries, please call us, at (719) 632-5700.
