Collaborative Clinic at Union

The purpose of the Collaborative Clinic at Union (CCU) is to provide an interdisciplinary approach to health care for persons living with HIV or AIDS. The goal of the CCU is to simplify the delivery of care and support treatment compliance. Peak Vista Community Health Centers patients are able to access the Collaborative Clinic at Union through a provider referral; additionally, patients can also be referred through the Southern Colorado AIDS Project.

Services:
- An initial visit with a primary care provider
- A co-visit with a primary care provider and an infectious disease specialist
- A visit with a case manager from Southern Colorado AIDS Project
- Assistance with ADAP (AIDS Drug Assistance Program)
- Assistance with housing and transportation
