If you have Medicaid, CHP+, Medicare or Private Insurance – click here to complete the registration form!

To apply for Medicaid or CHP+, click here to download an application.

If you do not have the above, call 632-5700 or walk-in to our Family Health Center at Fountain, Family Health Center at Divide, Family Health Center at Myron Stratton, Family Health Center at Union or our International Circle location.
