Our employee benefits are an important part of the total compensation package received by our employees. Unless otherwise specified, the term “full-time” refers to regular employees scheduled to work 30 or more hours per week. The term “part-time” refers to regular employees scheduled to work 20 to 39 hours per week. The following is a summary of the benefits offered at Peak Vista:
Medical and Dental Plan
Full-time and part-time employees are eligible for medical and/or dental benefits on the first of the month or after 60 days of employment. The company pays 100% of the cost of employee-only coverage of medical and dental benefits for full-time employees. The company contributes an additional dollar amount based on years of employment at Peak Vista if employee adds dependent coverage to medical and/or dental elections. Peak Vista will contribute a flat dollar amount per month toward the cost of medical and dental insurance for eligible employees working 40 hours or more after 36 months of employment.
Additional Vision Plan
Full-time and part-time employees are eligible for additional vision benefits on the first of the month or after 60 days of employment. Although Peak Vista’s medical plan includes vision benefits, employees may opt to pay for an additional vision plan.
In-Lieu Cash Benefit
Full-time employees who waive their option to enroll in the medical plan are eligible to receive a monthly cash benefit, once they show proof of coverage, on the first of the month or after 60 days of employment.
Flexible Spending Accounting (Medical and Dependent Care)
Full-time and part-time employees are eligible to enroll in our Dependent Care and/or Medical Flexible Spending Accounts. These plans allow you to reduce your taxable income by the amounts of anticipated dependent care expenses or for eligible medical expenses for you and your entire family up to IRS limits.
Qualified Retirement Savings Plan
Peak Vista Community Health Centers offers a Retirement Savings Plan to full-time, part-time (scheduled to work 1-39 hours per week), and temporary employees in which you are eligible to participate immediately. Employees can save up to annual IRS limits on a tax-deferred or post-tax basis. After one year of full-time or part-time employment, the company will match your deferrals dollar-for-dollar up to 4%. All deposits are 100% vested to the employee.
Full-time and part-time employees are eligible for various supplemental benefits on the first of the month following 30 days of employment. Colonial Life Supplemental Insurance provides various choices and they are all employee paid.
Group Term Life Insurance
Coverage from $10,000 to 5x annual salary with $100,000 guarantee issue (no medical questions). Dependent coverage is also available.
Group Term Short Term Disability
Pays 60% of income when you are unable to work.
Critical Illness Insurance
Individual and stand-alone Spouse plan available.
Employee and Dependent coverage available.
Employee and Dependent coverage available. Different levels of coverage to choose from.
Insurance policy is a hospital confinement indemnity plan that provides an employee with benefits for hospital confinement or outpatient surgery.
Pre-Paid Legal Services
All employees are eligible to purchase supplemental legal services from Pre-Paid Legal Services, Inc.
Health Education Classes
Classes will be held on a variety of health related subjects for a fee of $5.00 per class. These classes include a Diabetes Series, Asthma, Healthy Heart, and Healthy Weight Service.
Employees who have been at full-time status for one year are eligible to apply for education assistance reimbursement from an accredited university, school, or institution up to $1,500 per year.
Paid Time Off (PTO)
Paid time off is provided to full-time and part-time employees. This time can be used for vacation, personal leave, and illness. Paid time off is accrued each pay period that you work.
Full-time and part-time employees accrue long term leave based on the number of hours worked per week. When an employee is under the care of a physician, after 24 consecutive scheduled work hours of absence, long-term leave may be used for the additional time off for that episode of care.
Full-time and part-time employees are eligible for up to 40 hours of paid Bereavement Leave for the death of spouse, child, parent, parent-in-law, sibling, grandparent or miscarriage.
Peak Vista is closed in most departments on the following holidays: New Years Day, Martin Luther King Jr. Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Day after, Christmas Eve (half day), Christmas Day, and New Year's Eve (half day), and New Year's Day. Full-time and part-time employees receive pay for these company holidays.
On-Call Schedule (Providers Only)
Peak Vista's on-call schedule is shared among approximately forty providers to act as a back up to a telephone nurse triage system.
Life and Accidental Death and Disability Insurance
1.5 times salary up to $150,000
Long-Term Disability Insurance
60% of monthly salary up to $5,000
Full-time and part-time employees can apply for the loan repayment programs when available.
Employee Assistance Program (EAP)
Full-time and part-time employees are eligible for up to 3 counseling sessions per year, per person, per occurrence (includes employee, employee's spouse, and dependents of household up to age 23). The cost is paid by Peak Vista Community Health Centers.
Continuing Medical Education (Providers Only)
Up to one week of continuing medical education and reimbursement up to specified amount for eligible providers. Part-time providers are eligible for prorated continuing medical education.
Membership Dues (Providers Only)
Full-time and part-time providers can be reimbursed for up to 2 approved membership association dues.
