How do I enroll as a patient of Peak Vista?

There are two ways you can enroll as a patient of Peak Vista. You can call (719) 632-5700 or walk in to the health center closest to you. If you have Medicaid, CHP+ , Medicare or private insurance, click here to complete the enrollment form online. There is no waiting list for Medicaid and Medicare patients.

How much does it cost?

Unemployed and uninsured individuals are welcome, as well as those with private insurance, Medicaid or Medicare. If you do not have insurance, the fee for a visit is based on the size of your family and your total income and will be assessed using our sliding scale.

When will I get to make an appointment to see a physician/provider?

When you enroll, you will be asked to make your first medical appointment so you may get to know your physician/provider. We do have a waitlist but there is no waiting for seniors, pregnant women, children, Medicaid or Medicare patients. Some of our centers also have immediate appointments available - others will be called as soon as possible. For all appointment inquiries, contact our customer service department at (719) 632-5700.

HOW DO I MAKE A DONATION TO PEAK VISTA OR GET INVOLVED WITH THE ORGANIZATION?
